1.Public Health Leaders should embrace the role of Chief Health Strategist in their community. Driving initiatives including
those that address “upstream” social determinants of health.
2.Local Health Departments should engage with community stakeholders –from both the public and private sector –to
form structured, cross‐sectored partnerships.
3.Accreditation processes and criteria should be supported and enhanced to ensure that every person is served by an
accredited health department.
4.Timely, reliable, granular‐level (sub‐county), and actionable data should be accessible to communities with clear metrics to
document success.
5.Public Health funding should be enhanced and substantially modified with innovative funding models. Identify funding to
support core infrastructure plus community‐level work to address the social determinants of health.
