Board of Health (NHCBH) to order at 8:00 a.m., Wednesday, March 3, 1999, in the Dr. Thomas
Fanning Wood Memorial Conference Room, New Hanover County Health Department, 2029 South
17th Street, Wilmington, North Carolina.

Mr. Steuer asked for corrections to the minutes of the February 3, 1999 New Hanover County Board of
Health Meeting. The minutes of the February 3, 1999 Board of Health meeting were corrected and
approved.

Servin¡: Safe Food Certification
Mr. Rice recognized Mr. Mike Nance, ARA MARK, University of North Carolina @ Wilmington
(UNCW), who accepted a Serving Safe Food Certificate for UNCW. Five ARA MARK employees
were graduates of the Serving Safe Food Certification classes.
Mr. Rice thanked Ms. Jean Marsh and Ms. Isobel Charlton, Cape Community Fear College, for
helping NHCHD Environmental Health Division with the educational sessions. Ms. Marsh and Ms.
Charlton helped in administering the Serving Safe Food Certification classes.
Mr. Steuer welcomed new employees, commended the Service Award recipients, and Mr. Nance and
his fellow graduates for attending the Serving Safe Food Certification classes.

GeneratorslHookups at Emer¡:ency Shelters
Mr. Greer stated several months ago the county submitted a Grant Application to FEMA to provide
generators/hookups for emergency shelters in New Hanover County. He plans to keep the Board of
Health informed on the status of the grant application.
New Business:
Bud¡:et Amendment - Vector Control Boat
Mr. Steuer reported the Boat Committee decided readdress the boat safety issue before selecting a boat
for mosquito control. Also, on February 23, 1999, the Executive Committee again reviewed Boat
Options for the Vector Control Program.
Mr. Steuer asked Ms. Dianne Harvell, Environmental Health Director, to inform the Executive
Committee of a recent boating incident of the Vector Control staff. Ms. Harvell explained two staff
members encountered a wake from a large ship in the river and had difficulty keeping their boat in
positive floatation. Ms. Harvell and Mr. Steuer concurred the 16-foot boat currently used in the river
is inadequate. They recommended a larger boat and a crew of three for safety measures.
Mr. Steuer recommended from the Executive Committee for the Board of Health to purchase an
Alumaweld 19-foot boat ($19,065). This price includes a galvanized trailer, rigging, and shipping I
cost. A consideration in selecting the 19- foot boat was that the Coast Guard does not require flotation
of a boat 20 foot or more.

MOTION: Mr. Steuer moved from the Executive Committee for the Board of Health to accept and
approve the purchase of an Alumaweld 19-foot boat ($19,065) for the Vector Control Program in the
Environmental Health Division and to submit the budget request to the New Hanover County Board of
Commissioners for their consideration. Upon vote, the MOTION CARRIED UNANIMOUSLY.
Mr. Greer asked if the price for the boat includes all needed equipment. Mr. Steuer advised the boat is
fully equipment other than safety equipment. Mr. Estep asked ifthey could purchase the boat locally.
Mr. Steuer responded he did not find the heavy deep vee aluminum boat locally.
Grant Applications
HIV PREVENTION
Mr. Steuer recommended from the Executive Committee for the Board of Health to approve the
$190,000 Grant Application - Preventing HIV Effectively (PHIVE) - A Funding Proposal for Kate B.
Reynolds Charitable Trust Health Care Division. He stated Mr. Rice; Ms. Beth Jones,
Communicable Disease Director; and Mr. Michael McNeill, TAP Project Administrator; visited the
Kate B. Reynolds Division in Winston Salem, North Carolina, to learn more about the charitable
fund and the grant application process.
Mr. Steuer advised Ms. Jones and Mr. McNeill prepared the PHIVE Grant Application. The $190,000
funding proposal will enable the health department to assume leadership in coordinating HIV
prevention activities focusing on reaching the five highest risk populations.
MOTION: Mr. Steuer moved from the Executive Committee for the Board of Health to accept and
approve the $190,000 Grant Application - Preventing HIV Effectively (PHIVE) - A Funding Proposal
for Kate B. Reynolds Charitable Trust Health Care Division and to submit the PHIVE Grant
Application to the New Hanover County Commissioners for their consideration. Upon vote, the
MOTION CARRIED UNANIMOUSLY.
Dr. Goins explained the Board of Health adopted a grant procedure that requires the submission of a
grant application to the Executive Committee for recommendation and then to the Board of Health for
their consideration. If a grant deadline is relating to a budget amendment $10,000 or more (unless new
personnel request), the Board members will be notified of the additional monies and polled by phone.
Health Fee Policy Chan¡:e - Lymerix and Rabies Vaccines
Mr. Steuer recommended from the Executive Committee for the Board of Health to approve the
purchase of Lymerix vaccine (new vaccine) and the price increase of Rabies vaccine (pre exposure).
The requested changes to the New Hanover County Health Department Fee Policy are as follows:
Immunization Fees Cost Per Injection
Lymerix Vaccine (new) $55
Rabies Vaccine (pre exposure increase from $65 to $78) $78
MOTION: Mr. Steuer moved from the Executive Committee for the Board of Health to accept and
approve the purchase of Lymerix vaccine @ $55 per injection and the price increase of Rabies vaccine
(pre exposure) @ $78 per injection to be incorporated as changes to the New Hanover County Health
Department Fee Policy. Upon vote, the MOTION CARRIED UNANIMOUSLY.

Dr. Speck asked if the health department is reimbursed for expired vaccines. Ms. Jones explai~ed
vaccine fee charges are per injection and are based upon direct cost. The state does not refund ~xptred I
vaccine cost. Ms. Lynda Smith, Assistant Health Director, asked if they plan to advertIse the
availability of vaccines. Ms. Jones stated they plan to give vaccines to designated occupationally at
risk employees and plan to advertise the vaccines. Ms. Whitted noted Rabies vaccine was increased
$13.00 per injection. Ms. Jones stated the increase was due direct to cost of Rabies vaccine.
Medicaid Rate Increases
Mr. Steuer recommended from the Executive Committee for the Board of Health to approve the
Medicaid rate changes submitted.
Mr. Greer and Mr. Estep asked if Medicaid rates change each year. Mr. Rice explained the state
notifies the health department of rate changes. Dr. Goins added Medicaid does not have cost of living
increases annually.
Ms. Smith, stated Medicaid rate increases will be presented to the Board of Health and then to the New
Hanover County Commissioners for their consideration. She advised health department fees cannot be
less than Medicaid charges. They will submit other fee changes late!.
MOTION: Mr. Steuer moved from the Executive Committee for the Board of Health to accept and
approve to the Medicaid Rate Increases to be incorporated in the New Hanover County Health
Department Fee Policy and to be submitted to the New Hanover County Commissioners for their
consideration. Upon vote, the MOTION CARRIED UNANIMOUSLY.
Grant Applications - Smart Start
I
INTENSIVE HOME VISITING PROGRAM (IHVP)
Mr. Steuer recommended from the Executive Committee for the Board of Health to approve a Grant
Application - INTENSIVE HOME VISITING PROGRAM (IHVP) - Partnership for Children 1999-
2000. The funding proposal is for $100,000. It will fund 3 positions - 1 social worker, 1 nurse, and 1
part-time clerk (50%).
MOTION: Mr. Steuer moved from the Executive Committee for the Board of Health to accept and
approve to $100,000 Grant Application - INTENSIVE HOME VISITING PROGRAM (IHVP) -
Partnership for Children 1999-2000 Smart Start Funding and to submit the IHVP Grant Application to
the New Hanover County Commissioners for their consideration. Upon vote, the MOTION
CARRIED UNANIMOUSLY.
HISPANIC PRENATAL PROJECT
Mr. Steuer recommended from the Executive Committee the approval of a Grant Application -
HISPANIC PRENATAL PROJECT - Partnership for Children 1999-2000. The funding proposal is
for $12,700 grant is for Hispanics who are illegal aliens.
Ms. McCumbee explained some pregnant women are illegal immigrants; however, once the child of an
illegal alien is born, the baby becomes a legal citizen of the United States and a financial responsibility
of New Hanover County. She advised the proposed grant will target barriers to prenatal care and will I
address transportation, prenatal vitamins, diabetic supplies, car seats, interpreter services, motherlbaby
supplies, and prenatal support education sessions. Ms. McCumbee emphasized better prenatal care
produces healthier babies.

Mr. Steuer expressed he has a problem with illegal aliens having children and the babies becoming
legal citizens and the responsibility of the county. Ms. Whitted emphasized we need to provide public
health care to these people. She stated the laws are a government problem that the Board cannot
change. Mr. Steuer agreed these people need health care; however, the citizenship laws need to be
addressed and changed.
Mr. Greer concurred with Mr. Steuer this is a problem in our community that needs to be addressed
and that the citizenship laws need to be changed. He expressed it is wrong for illegal aliens to come to
the United States, have children, and then expect local taxpayers to pay expenses. Mr. Greer
suggested the Board of Health prepare a letter to congressmen stating it is a problem when local
government is expected pay the medical expenses for illegal aliens who have children who
automatically become United States citizens.
MOTION: Mr. Steuer moved from the Executive Committee for the Board of Health to accept and
approve the $12,700 Grant Application - HISPANIC PRENATAL PROJECT - Partnership for
Children 1999-2000 Smart Start Funding and to submit the grant application to the New Hanover
County Commissioners for their consideration. Upon vote, the MOTION CARRIED
UNANIMOUSLY.
MOTION: Mr. Greer MOVED, SECONDED by Ms. Whitted for the Board of Health to send a letter
(individually signed) to our congressmen explaining the problems of children born to illegal aliens in
our community, of these children becoming United States citizens at birth, and of local government
being expected to pay their expenses. Upon vote, the MOTION CARRIED UNANIMOUSLY.
Bud¡:et Amendment - Medicaid Maximization
Mr. Steuer recommended from the Executive Committee for the Board of Health to approve the
Budget Amendment for receipt of$151,703 Medicaid Maximization Funds from the N. C. Department
of Health and Human Services.
Ms. Smith briefly discussed the proposed use of the Medicaid Maximization Funds. Mr. Estep asked
if we will receive Medicaid Maximization Funds every year. Ms. Smith stated the health department
received the Medicaid Maximization Funds for the first time last year and again this year. She is not
certain of the frequency of the funding.
MOTION: Mr. Steuer moved from the Executive Committee for the Board of Health to accept and
approve the $151,703 Medicaid Maximization funds and to submit the budget amendment to the New
Hanover County Commissioners for their consideration. Upon vote, the MOTION CARRIED
UNANIMOUSLY.
Resolution in Support of Environmental Health
Mr. Steuer recommended from the Executive Committee for the Board of Health to approve a
Resolution in Support of Environmental Health Remaining in the North Carolina Department of
Health and Human Services (DHHS).
Mr. Steuer explained in 1997 the Board of Health approved the "WHITE PAPER, a Resolution in
Support of Bringing Environmental Health Programs in the Department of Health and Human
Services (DHHS). He referred the Board to a DHHS Organizational Chart with the Division of
Environmental Health under the State Health Director and the DHHS. The Board of Health reviewed
the proposed resolution in support of Environmental Health remaining under DHHS.

MOTION: Mr. Steuer moved from the Executive Committee for the Board of Health to accept and
approve a Resolution in Support of Environmental Health Remaining in the North Carolina
Department of Health and Human Services and to submit the resolution to the New Hanover County I
Commissioners for their consideration. Upon vote, the MOTION CARRIED UNANIMOUSLY.

Comments:
Board of Health
Communicable Disease Trainin¡: Requirement for Licensed Professionals
Dr. Goins reported he attended a professional continuing education session on AIDS and Hepatitis in
Florida. As a licenced operator, they require that he have one hour of training in AIDS education.
He stated these are public health problems and that he endorses a training requirement for licensed
operators. Dr. Goins suggested researching a requirement of this type for licensed professional people
in the health department.
Mr. Rice suggested he work with Ms. Beth Jones on this Communicable Disease issue. He stated they
could research the continuing education program in Florida and address the issue of a training
requirement for professionals.
Mr. Steuer requested Mr. Rice and Ms. Jones study a training requirement for licensed operators on
the broader spectrum of communicable diseases. The Board of Health concurred with Mr. Steuer's
request for Mr. Rice and Ms. Jones to address this educational issue for licensed people.

Trauma Symposium - Injury Prevention and Control
Mr. Rice reported he presented the welcome at the Trauma Symposium on Injury Prevention and
Control held at UNCW on February 13, 1999. Dr. Thomas C. Clancy, former Board of Health
member, invited Mr. Rice to participate in the symposium. Approximately, 400 people attended the
injury prevention symposium.
New Hanover County Board of Commissioners Presentation
Mr. Rice advised he gave a presentation on the New Hanover County Health Department Annual
Report and the North Carolina Public Health "Everywhere, Everyday, Everybody" video at the County
Commissioners meeting Monday, February 15, 1999. At Mr. Greer's suggestion, the County
Commissioners requested Mr. Rice arrange to show the video on local and on public television. Mr.
Rice plans to develop a list of public health issues and activities. Mr. Rice stated Mark Boyer, Public
Information Officer, is working with him to obtain media coverage including local and public
television.
Child Passenger Safety - "Click It or Ticket"
Mr. Rice stated a Child Passenger Safety - Click It or Ticket was held Wednesday, February 17, 1999, I
at the Temple Baptist Church Activity Center to promote correct usage of child safety seats. Local
law enforcement, Smart Start and the Governor's Highway Safety Program, and the Health
Department participated in the checkpoint for child safety seats. Approximately 25 seats were
checked, and they found 17 seats were incorrectly installed.

Criminal Justice Discussion Group
Mr. Rice stated he participated in a Criminal Justice Discussion Group on February 26, 1999. District
Court Judge Rebecca W. Blackmore gave a presentation on Drug Court.
Mid-Atlantic Mosquito Control Association Annual Meetin¡:
Mr. Rice announced today he is presenting the Welcome at the Mid-Atlantic Mosquito Control
Association Annual Meeting. The annual meeting will be held March 3-5, 1999, at the Blockade
Runner, Wrightsville Beach, North Carolina.
UNCW Adult Scholars Leadership Program
Mr. Rice stated the Board the Adult Scholars invited him to speak at a UNCW Adult Scholars
Leadership Program on March 11, 1999.
1999 Sprin¡: Blood Drive - March 16. 1999
Mr. Rice invited Board members to participate in the County 1999 Spring Blood Drive on March 16,
1999, at the health department. Ms. Smith reminded the Board members they may give blood before
and after the Blood Drive at the Red Cross Center on 17th Street.
April 7. 1999 New Hanover County Board of Health Meeting at Animal Control Services
Mr. Rice announced the Board of Health will meet at the Animal Control Services building at 8:00
a.m. on April 7, 1999. Notices of change of location meetings will be placed on the April Board of
Health agenda and packets.
Other Business:
Euthanization Video
Mr. Steuer announced he will show a video on Euthanization made by the Guilford County Sheriff at
the April 7 Board of Health meeting. He suggested the Board may want to arrange to show the video
on local and public television.
Board of Health Meeting Convened from Regular Session to Closed Session:
Mr. Steuer advised the Board they need a motion to close the public meeting to consider a personnel
matter as required by North Carolina General Statute Section G.S. 143-318.11, Subsection a.,
Subsection 6.
MOTION: Mr. Greer MOVED, SECONDED by Dr. Smith to close the public meeting of the New
Hanover County Board of Health to consider a personnel matter as required by North Carolina
General Statute Section G.S. 143-318.11, Subsection a., Subsection 6. . Upon vote, the MOTION
CARRIED UNANIMOUSLY.

IN SUPPORT OF PLACING ALL PROGRAMS
TRADITIONALLY
IN THE DIVISION OF ENVIRONMENTAL HEALTH
UNDER THE ORGANIZATION OF
DEPARTMENT OF HEALTH AND HUMAN SERVICES
WHEREAS, delivery of Environmental Health services to the Citizens of North Carolina is carried out at
the county level by local health department employees; and
WHEREAS, delivery of Environmental Health services at the local level is funded by local dollars (97%
local and 3% state); and
WHEREAS, Environmental Health services at the local level include on-site wastewater (i.e. septic
systems) food, lodging and institutional sanitation (restaurants, nursing homes, food borne disease
investigation) child care sanitation, child lead program, swimming pools, rabies control, safe drinking
water; and
WHEREAS, the State relies on local employees who are authorized as agents of the State Division of
Environmental Health to carry out mandated State environmental health programs; and
WHEREAS, having two different proposed State environmental health authorities for local health
departments is inefficient and will cause regulatory confusion for the public and severely impact centralized
training, authorization, regulatory direction and uniformity; and
WHEREAS, the N.C. Association of County Commissioners, the N. C. Association of Local Health
Director's, the Association of North Carolina Boards of Health, the N. C. Environmental Health Supervisor's
Association, and the N. C. Public Health Association, have endorsed the position to keep environmental
health programs together under the State Health Director;
THEREFORE BE IT RESOLVED, the New Hanover County Board of Health strongly supports the need
to ensure all aspects of Environmental Health be under the organizational structure of the Department of
Health and Human Services in order to continue to provide the most effective regulatory program for the
citizens of North Carolina for the protection of the public health and preservation of the environment.
