NEW HANOVER COUNTY BOARD OF COMMISSIONERS REQUEST FOR BOARD ACTION MEETING DATE: September 19, 2011 CONSENT ITEM: 3 DEPARTMENT: Health PRESENTER(S): Janet Smithey, Personal Health Services Manger CONTACT(S): Janet Smithey, Personal Health Services Manager SUBJECT: Ratification of Participation in Diabetes Program Grant Application Submission with the Wilmington Southside Community Development Corporation (WSCDC) BRIEF SUMMARY: The WSCDC requested our participation in a Kate B. Reynolds (KBR) Grant submission for September 15, 2011. WSCDC is applied for funds to provide diabetes outreach, screening, education and self management classes to residents in specific low income housing areas of Wilmington, such as the Wilmington Housing Authority's (WHA) Jervay Loop, Hillcrest, and Houston Moore developments. US Census data show that 30% of the population in these target areas lives below poverty. Furthermore, given that 100% of the target population resides in public housing, all of the participants would be considered financially needy, and uninsured or underinsured. The New Hanover County Health Department (NHCHD) Diabetes Program was started in October 2010 with a grant from the Office of Minority Health, which will end in October 2011. In July of 2011, our diabetes program was granted certification by the American Diabetes Association (ADA) under the "umbrella" of NC Diabetes Education Recognition Program (NC DERP). This certification will allow billing for self management classes. This new opportunity in partnership with WSCDC would afford us the opportunity to continue our program, providing much needed services to low income citizens with diabetes or at risk for diabetes. No County match is required. If the grant is awarded, two (2) half -time temporary positions would become two (2) new part -time regular positions (.5 FTE each) with benefits. If the KBR grant is not awarded, the existing Office of Minority Health grant funded part -time regular administrative position and two temporary positions will be eliminated when that grant funding ends. Those three positions are a registered dietician, a public health nurse and a fiscal support technician. Standfest 8/17/11 Revised 8/22/11 Diabetes Program- August 2011 In New Hanover County (NHQ, diabetes ranked high among the common health indicators and leading causes of mortality (Behavioral Risk Factor Surveillance System, 2009). The increase is largely in minority groups. The mortality rate related to diabetes ranked first in the state with the prevalence being 23.8 percent non - Hispanic white males, 65.6 percent minority males, 11.3 percent non - Hispanic white females and 65.7 percent minority females (NC Diabetes Prevention and Control Fact Sheet, 2009). New Hanover County (NHC) is geographically located in the southeastern region of North Carolina. New Hanover County is the second smallest county, yet, one of the most densely populated counties in the state. The current race /ethnicity of New Hanover County consists of 81.7 percent non - Hispanic white, 15.6 percent African American, 3.5 percent Hispanic /Latino, and 0.4 percent American Indian /Alaskan Native. Over the past few years, New Hanover County has seen increases in unemployment and poverty rates. In 2006, the number of persons living below poverty was 14 percent in New Hanover County. Socioeconomic conditions have led to an increase in uninsured and underinsured residents. This has placed a great demand for access to free or low cost healthcare and preventive medicine in communities surrounding NHCHD. The New Hanover County Health Department is one of the oldest health departments in the state. We are a state accredited health department and provide quality health care, preventive medical, mental health and dental services, as well as environmental protection for New Hanover County citizens. The NHCHD implemented a diabetes prevention and self - management program in 2010, under the guidance of NC Diabetic Education Recognition Program (NC DERP), to service African American and Hispanic population groups. Goals and objectives of the Diabetes Program were developed with guidance from The Standards and Procedures for Diabetes Self - Management Education, North Carolina Diabetes Education Recognition program (recognized under the American Diabetes Association umbrella program), and evidence -based strategies for best practices derived from review of credible published literature on diabetes studies will be followed. Goals focus on reducing barriers to healthcare by providing health screenings, Board of Commissioners Meeting "Healthy People, Healthy 9 � n Healthy Community" referrals, and follow -up care; reducing the prevalence of reported cases of diabetes in NHC; increasing awareness of the disease and related complications; and establishing a coalition of community partners to assist in combating Type II diabetes. NHCHD has been strong in developing and maintaining cooperative partnerships with many local organizations and safety net providers, such as Tileston Clinic, Cape Fear HealthNet, Lower Cape Fear - Access III, New Hanover Regional Medical Center, Cape Fear Hospital, and the faith -based community. Furthermore, our working relationship with the media is excellent and one that serves us well with regard to health messaging and improving the lives of our citizens. We use the New Hanover County Public Information program to develop and foster messages of prevention and engage the community in the delivery of programs around prevention. We are also able to produce our own programming to air on New Hanover County Television. The New Hanover County Health Department has past and current experience in working with diabetes education and management initiatives. In 2002 -2005, our health department managed a grant funded program, called Diabetes Today, which reached over 358 individuals with diabetes. Some of the successful objectives accomplished by this program included: 1) participants had increased knowledge of how to manage diabetes and ways to prevent the complications of uncontrolled diabetes, 2) augmented the percentage of diabetics receiving recommended foot and eye exams, 3) provided multiple venues in which people with diabetes and those at risk could take control of their health with support, education and healthy lifestyle programs, and 4) successfully enrolled members of the Latino population who then recruited friends and family members to join in the educational and prevention programs offered in our community. The NHCHD was awarded grant funding October 2010 by Office of Minority Health and Health Disparities (OMHHD) to re- implement a diabetes program. While NHCHD has made great strides in establishing the new diabetes program, we are now facing the potential loss of funding from OMHHD, due to legislative mandates. In July of 2011, our diabetes program was granted certification by the American Diabetes Association (ADA) under the "umbrella" of NC Diabetes Education Recognition Program (NC DERP). Under NC DERP the diabetes program is set -up to track monthly success rates of participants enrolled in the diabetes education classes. In adults, Type II diabetes accounts for about 90 to 95 percent of all diagnosed cases of diabetes. As a result of the prevalence, costliness, and catastrophic impact of Type II diabetes upon those affected, more specifically, African Americans and Hispanic population. Diabetes and its related complications often can be controlled with appropriate health care and patient self - management Board of Commissioners Meeting "Healthy People, Health n Healthy Community" techniques (US Dept of Health and Human services, 2004). The NHCHD diabetes program offers prevention strategies that are culturally specific and relevant, and encompasses the provision of outreach educational and screening services. The diabetes program is designed to educate people, in high risk population, that implemented lifestyle changes reduced their risk of getting Type II diabetes. NHCHD preventive strategies is inclusive of having open group discussion on making wise and healthy food choices, exercising regularly, controlling blood pressure and cholesterol, and seeking medical treatment with diabetes medicines, when absolutely necessary. People who lost an average of 15 pounds reduced their risk by 71 percent. People controlling their glucose levels through use of diabetes medications, reduced their risk by 31 percent. Furthermore, dietary intervention, weight management, physical activity, control of cholesterol and blood pressure are critical components of diabetes self - management and prevention of microvascular and macrovascular complications associated with diabetes, such as retinopathy, renal failure, cardiovascular disease, amputation of limbs, etc. While medical resources and healthcare might be available in NHC, they are often inaccessible, unaffordable, or under - utilized, unknown, or generally mistrusted by the targeted population. New Hanover County community has come to trust and rely on NHCHD to provide quality healthcare services. Since inception in October 2010, the NHCHD diabetes program has screened a total of 310 participants with 62 participating in the diabetes self - management classes. To date, the NHCHD diabetes self - management classes had 13 participants to complete education classes and follow -up assessment, and a 38 percent success rate of participants with improve hemoglobin Al C blood test (which is an indicator of their diabetes control). Our diabetes program has a proven success record, as evidence by the following Diabetes Program Success Story — A 44 -year old male who was under - insured requested help with his diabetes. He had used a relative's glucose meter and found his blood sugar was 465. His vision was deteriorating and he was frightened. He attended the health department's February Diabetes and Heart Awareness outreach; at which time he was referred to and seen at Cape Fear Health Net, started on medications, and was referred to the NHCHD diabetes self management classes. In February (before attending class), his Hgb A 1 C blood test was 9.3 (goals is <7.0) and had dropped to 5.4 at the three month follow up visit. His fasting blood glucose was 107 and he has stopped smoking. NHCHD has been accredited since 2004, which validates the high standards of public health services provided by a first -class health department. The NHCHD has also participated in several strategic planning processes over the past six years, identifying the greatest needs in the community. One of the top goals of our department is "Access to Care ". We continue to strive to meet the health care needs of the citizens of New Hanover County and /or ensure access to care for low income and uninsured citizens (14% of the population is under the poverty level). Diabetic supplies for uninsured pregnant mothers have been identified by the Health department staff as a significant need. The Pregnancy Care Management Program (PCM), formerly known as the Maternity Care Coordination Program, began on March 1, 2011. The program consist of prenatal care providers who agree to serve women who receive Medicaid, private insurance and uninsured, throughout their pregnancy and up to two months postpartum. The Pregnancy Medical Home (PMH) provides access to prenatal care, a stable relationship with one health care provider for their entire pregnancy, a Pregnancy Care Manager (PCM) to work with them if their pregnancy is considered high risk, 24 -hour telephone access to a health care professional for questions or concerns, and referrals to other programs such as Medicaid, WIC, family planning, and other local resources as needed. PMH offices include the Coastal Ob /Gyn clinic at New Hanover Regional Medical Center and Coastal Family Medicine Clinic. Pregnancy Care Managers, who are RNs or Social Workers, will help patients identify their needs such as substance abuse, domestic violence, and homelessness and make plans to address them, work closely with the patients and their doctors to understand their needs, and help arrange transportation to medical appointments if needed. The PCM program is under the supervision of Panza A. McNeill, RN. Gestational diabetes mellitus (GDM) is defined as any degree of glucose intolerance with onset of first recognition during pregnancy. Approximately 4% of all pregnancies are complicated by GDM. However, the prevalence may range from I% to 14% of all pregnancies, depending on the population studied. The presence of fasting hyperglycemia may be associated with an increase in the risk of intrauterine fetal death during the last 4 -8 weeks of gestation. Furthermore, GDM increases the risk of fetal macrosomia and other neonatal morbidities including hypoglycemia. GDM is associated with an increase frequency of maternal hypertensive disorders and the need to the fetus and this is best accomplished through careful monitoring and the use of insulin if indicated. This cannot be accomplished without the needed supplies and medications being made available to every woman who is in need of them. Among other improvements, the team has produced an instructional video for nurse training in order to sustain the continued consistency in drawing doses from multi -dose vials. This quality improvement project has been selected to be highlighted by the North Carolina Center of Public Health Quality as an example for other local health departments in the state and has drawn interest from the Centers for Disease Control in Atlanta, Georgia as well as the State of North Carolina's Immunization program. This is the second NCACC Productivity award for NHCHD in recent years. In a county surrounded by water like New Hanover, chances are good that it will end up in the river or the ocean fairly quickly. Once it hits the water, litter becomes marine debris - one of the most preventable sources of pollution in our oceans today. On Saturday, September 24, 2011 hundreds of volunteers throughout New Hanover County will gather to reduce the amount of litter that becomes marine debris during Big Sweep, an annual cleanup of our waterways. Big Sweep will begin at 9:00 am in Wrightsville Beach at the Gazebo at Johnnie Mercer's Pier, Carolina Beach at the Boardwalk Gazebo, Kure Beach at the Pier, and Greenfield Lake at the Boathouse. Volunteers are needed! Please come prepared to be outdoors: wear sunscreen, sturdy closed toe shoes and clothes that can get dirty. Help us reduce the amount of waste at the event by bringing your own reusable water bottle and gloves if you have them. Big Sweep began as a beach sweep in 1987 and is now an annual statewide event. The request was approved in the amount of $270,000. The purpose of the grant funding was to purchase this equipment to be installed on the Sheriff's Office helicopter; however, after submission of the grant application and prior to grant award, the County's helicopter was sold. The Sheriff's Office requested and was given approval to keep the grant funds and to use the money to purchase and install the equipment on the City of Wilmington's SABLE helicopter. The City currently has two helicopters and the first helicopter is equipped with a Broadcast Microwave Service System, an AeroComputers Mapping System and a FLIR System which are all part of the downlink system. Staff is requesting that AeroComputer and Broadcast Microwave Services, Inc. (BMS) be approved as sole source vendors for purchase of an Inertial Measurement Unit from AeroComputer and BMS Microwave system from BMS, Inc.; to be installed on the second SABLE. This will ensure compatibility and that the equipment will integrate with the current equipment that is already installed on the first SABLE Helicopter. The County will retain ownership of the equipment and if the SABLE helicopter is sold, the equipment must be returned to the County. A Memorandum of Understanding has been executed between the County and the City regarding this equipment. General Statutes 143- 129(e)(6) provides that purchases of apparatus, supplies, materials or equipment are exempt from bidding when: A. performance or price competition for a product is not available. B. a needed product is available from only one source of supply. C. standardization or compatibility is the overriding consideration. This exception to the bidding requirements is referred to as "Sole Source" and must be approved by the Board prior to purchase. The total cost of the sole source equipment is $174,334.
