be determined where applicable. Individuals who are identified as possibly eli-
gible will be referred to appropriate agencies for eligibility determination.
Once eligibility for reduced rates is determined, it will be continued for a
period of one year, unless .otherwise requested by a patient. Third party
coverage will be verified on each visit.
Fees for environmental health and animal control serV1ces will not be adjusted
by the sliding scale.

VI. Collection
Fees will be collected at the time service is rendered, by trained clerical per-
sonnel at central bookkeeping stations. In certain cases, fees will be collected
at the point of service by the professional rendering serv1ces. Responsible audit
trails will be maintained for all fee collections.
Individuals who pay at the time of service will be provided with itemized receipts.
Itemized bills will be sent to others,monthly. Enrollment in a third party pay-
ment plan may be presumed to constitute full payment for that service.
Failure to pay a proper charge for services rendered will constitute a debt for
reasonable collection. Unpaid accounts will be bil~ed monthly. Balance due
of less than five dollars ($5.00) will be billed ~g~~. Balances of $5.00
to $25.00 will be billed three times. Balances in excess of $25.00 will be
billed four times. Third and fourth bills will be stamped "Account Past Due,
Service May be Withheld Pending Payment." Balances less than $2.00 will
not be billed.

ORes a yaar, the Health Director, or his designated representative, will review
bad debts outstanding six months after the last bill has been rendered as described.
At that time he or she will authorize write off of such amounts for accounting pur-
poses. Payments received later for bad debts will be accepted and credited to..
appropriate accounts. However, no further collection efforts will be applied to

VII. Delinquent Accounts
No individual will be denied services at the New Hanover County Health Department
because of inability to pay, as determined by the Sliding Fee Scale.
With the exception of Family Planning, patients, individuals with unpaid balances
may be denied subsequent optional services, pending demonstration of a good faith
effort to make payment.

RESOLUTION ACCEPTING OFFERS TO
PURCHASE SURPLUS REAL PROPERTY
WHEREAS, cert~in offers to purchase parcels of real property owned wholly or
jointly by New Hanover County and/or the City of Wilmington have been advertised
for upset bid in compliance with NCGS 160A-269 and pursuant to resolutions
adopted by the City Council of the City of Wilmington on July 27, 1982, amended
on August 10, 1982 and the Board of Commissioners of New Hanover County on
August 2, 1982, amended on August 16, 1982; and
WHEREAS, there have been no further qualifying upset bids entered for the
properties described on the attached list within the statutory period allowed
by NCGS 160A-269; and
WHEREAS, the offerors indicated on the att.a.r.-..hed list have agreed to pay the
amounts indicated on the attached list for the parcels of real property shown
thereon; and
WHEREAS, the properties are declared by the County Commissioners to be
surplus and not needed for public purposes; and
WHEREAS, it is in the best public interest and welfare to dispose of these
properties in accordance with the negotiated terms and conditions.
NOW THEREFORE BE IT RESOLVED:
1. That pursuant to NCGS 160A-269, the County Commissioners do hereby
accept the offers to purchase indicated on the attached list from
the offeror as indicated.
2. That the City of Wilmington and/or New Hanover County will retain
any deposit posted by the offerors as liquidated damages if the
offeror fails to pay the balance of an approved offer due in
cash within ten (10) days of receipt of a notice by certified
mail of availability of the deed of conveyance.
3. That the Chairman of the Board of Commissioners of New Hanover
County be and is hereby authorized to execute a deed without
warranties to the offerors as designated on the attached list.

BY-LAWS OF THE NBv HANOVER COUNTY IN-HOME SERVICES COMMITTEE
ARTICLE 1 - NAME
The organization shall be known as New Hanover County In-Home Services
Committee
ARTICLE II - PURPOSE
The purpose of the committee shall be:
1. To improve in-home services to the fifty plus (50+) population of
New Hanover County.
2. To improve access to in-home services.
3. To improve communication between the service providers.
4. To advocate for the provision of needed in-home services.
ARTICLE III - GOALS AND OBJECTIVES
1. To promote a comprehensive continuum of in-home services to the
fifty plus (50+) population of New Hanover County.
A. To provide an inter-agency forum to inform the service
providers of all of the services that exist for the fifty
plus (50+) population.
B. To identify existing gaps in services.
C. To explore possible means of filling gaps.
D. To educate the community about the continuum of care.
E. To provide a forum for service providers to problem solve
difficult cases.
2. To minimize duplication of services
A. To identify existing duplication of services.
B. To be certain that any duplication of services is necessary
in the interest of clients and that service providers have
the needed information in regards to clients so they can
coordinate their services with others.
C. To provide coordination so the service providers will be
aware of appropriate service referral sources as client's
need change.
3. To recommend to funding sources the need for in-home services and
where funding priorities should be directed.
A. To identify existing in-home services to community.
B. To identify and prioritize gaps.
C. To advocate for in-home services monies on the pr ivate~
local~ State and Federal levels.

All committee members have full voting rights. Any substitute sent to
committee meetings for absent members are permitted full voting rights. It
is recommended that all committee members serve for a minimum of one year.
The decision to increase or reduce the size of the present membership shall
rest With the full committee.
ARTICLE V - OFFICERS
The officers of this committee shall be as follows:

TERMS
Officers shall serve for the duration of one (1) year from July through
June. Officers may succeed themselves to a maximum of two consecutive
full terms. At the July meeting of the full committee, officers shall
be elected from among the members by a majority vote with the
membership present. Notice of upcoming election and proposed slate of
officers shall be mailed to each current member no less than two weeks
prior to said election.
VACANCIES
When vacancies occur among officers because of death, resignation,
inability to serve, the full committee shall appoint a person to serve
until expiration of the remaining term of office. If the chairman
position should become vacant, the Vice-chairman shall automatically
fill the vacancy. Other officers shall be appointed by committee to

ARTICLE VI - POWERS AND DUTIES OF OFFICERS
A. The chairman shall:
1. Conduct regular monthly meetings
2. Call and conduct any additional meetings that may be deemed
necessary
3. Carry out necessary correspondence, communication and reports
on behalf of the full committee.

4. Appoint such special committees as may from time to time to
be necessary, and may serve as a member of said committee.
a. Prior to June, 1983, and subsequent to approval of ~hese
By-Laws, the Nominating Committee will present the slate
of officers for election.
b. Beginning in June, 1983, a Nominating Committee shall be
appointed each year at the June meeting to bring up the
full slate of offices before the membership for election
at July meeting.
5. Maintain in an orderec;i way and keep safe from harm the
records of current proc~edings as well as those of preceding
years.
6. Perform other duties as may be specifically delegated to him
by concurrence of the full committee.
B. The Vice-Chairman shall exercise the duties of the chairman in his
absence having the same authority as the chairman.
C. A Chairman Pro-Tempore may be appointed by the committee if both
Chairman and Vice-Chairman are absent.
D. The Secretary shall:
1. Keep the minutes of all proceedings of the committee meeting
2. Transcribe said minutes and within two weeks mail a completed
copy to each committee member who was present at that meeting
or to members who request the minutes of that particular
meeting.
3. Keep the records of committee appointments, officers' list~
attendance records, list of members showing their addresses,
telephone number, and term of office.
4. Send to each committee member notification of the time, date,
and place of the up-coming meeting and copy of the proposed
agenda no less than ten (10) days prior to meeting.
5. Perform other duties in regards to records and correspondence
and arranging for meeting site as delegated to him by the
full committee or chairman.
D. The Executive Committee composed of Chairman, Vice-Chairman, and
Secretary shall act on matters pertaining to business between
committee meetings.
ARTICLE VII MEETINGS
1. Time for holding meeting:
There shall be a monthly committee meeting and special meetings
may be called by the chairman, when necessary.
2. Quorum:
The business at the regular meeting shall be carried out by those
members present with full authority to act.
3. Order of business: Committee conduct shall be as laid down in
Robert's Rules of Order (revised).
4. Closed meetings will only be held to discuss individual cases and
for other reasons of confidentiality.

ARTICLE VIII DUES

Expenses of the Committee will be covered by an Annual Dues Assessment
agreed upon by the Committee.
ARTICLE IX - AMENDMENT
These By-Laws may be amended at any regular meeting of the committee.
