Project Title:
G.R.E.A.T. Summer Program
The following terms are agreed upon as conditions for this grant:
1. The tax - exempt status verified in the proposal (if required) is still valid; any changes in the organization that
could lead to a change in this status will be reported to the Good Shepherd Fund immediately.
2. The funds will be used by the above -named organization solely for the approved purposes described in the
project title above. No funds will be utilized for the purpose of promotion of advancement of political causes.
3. The organization will keep and maintain records of expenditures adequate to verify the use of grant funds.
Progress reports concerning budget and program will be completed and returned to Good Shepherd Fund (a
check register will not suffice) by the specified deadline. Misappropriation of grant funds or unsatisfactory
progress (as determined by the Fund) may result in a demand that the grantee repay grant funds to the Good
Shepherd Fund.
4. The organization will repay, upon demand, to the Good Shepherd Fund the amount of the grant if any
condition of the contract is not upheld.
5. The organization will send to the Good Shepherd Fund copies of one printed publicity article regarding the
award of the grant to the program supported by the grant.
6. All required reports will be submitted by November 30 of award year and a presentation at the November
special meeting for grantee reports.
7. Any changes including change in Executive Director, Contact Person, or Fiscal Officer will be reported to The
Good Shepherd fund immediately.
SPECIAL CONDITIONS:
1. Your budget is approved as follows:
T -shirts $1,400. 00
(No food or refreshment allowed)
Total Grant $1,400.00
2. Your grant funds can only be spent on above materials or their equivalent.
No food, refreshments or utility bills are allowed with funding proceeds.
