Addendum 1
The terms set forth in the Family Caregiver Support Program Grant Agreement under III.
General Provisions Category 4 Respite Services (842) are amended to expand the per
caregiver amount from $1,500 to $2,000 within the fiscal year. And the terms set forth
under Category 5 Supplemental Services (850s) are amended to expand the per caregiver
amount from $500 to $800 within the fiscal year. These amendments are approved through
the end of the contract June 30, 2019.
IN WITNESS THEREOF, THE COUNCIL OF GOVERNMENTS AND THE GRANTEE HAVE
EXECUTED THIS AGREEMENT AS OF THE DATE FIRST WRITTEN ABOVE.

Please see the attached addendum to your current Family Caregiver Support Program Grant Agreement. We have increased the
per caregiver amounts in Category 842 Respite Service category and the 850s Supplemental Services categories. We have found
after conversations with many of you that caregivers need a little more support in these two categories.
Please ask your director to sign and return the attached addendum to me. I will acquire the proper signatures here and return a
fully executed copy to you.
