Home and Community Care Block Grant for Older Adults
Amendment 1 to Agreement for the Provision of County -Based
Aging Services
July 1, 2020 through June 30, 2021
This AMENDMENT is made to the Home and Community Care Block Grant for
Older Adults Agreement for the Provision of County -Based Aging Services, July 1,
2020 through .lune 30, 2021 (the "Agreement") by and between the County of New
Hanover (hereinafter referred to as the "County") and the Cape Fear Council of
Governments, Lead Regional Organization for the Area Agency on Aging
(hereinafter referred to as the "Area Agency") dated the 4th day of November 2020.
1, Paragraph 4 is amended as follows:
Services authorized through the County Funding Plan, as specified on the
Provider Services Summary (DAAS-732) Revision 4 dated July 15, 2021,
represented here as Attachment A, are to commence no later than July 1 of the
state fiscal year and shall be undertaken and pursued in such sequence as to assure
their expeditious completion. All services required hereunder shall be completed
on or before the end of the Agreement period, .lune 30 of the state fiscal year.
2. Paragraph b is amended as follows:
Compensation and Payments to the County. The County shall be compensated
for the work and services actually performed under this Agreement by payments
to be mads: monthly by the Area Agency. Total reimbursement to the community
service providers under this Agreement may not exceed $1,223,622.
