Reagan Electrical - Change Order lIE - in the amount of $3,281.00 - This change order covers providing motor starter 3/4 inch conduit, disconnect, wiring and other materials necessary to provide a complete installation for EF 6 fan. Reagan Electrical - Change Order 12E - in the amount of $12,732.00 - This change order covers additional wiring, fire alarm upfit for smoke detectors and additional circuits. Sneeden, Inc - Change Order 13M - in the amount of $1,393.00 - This change order covers installation of tamper proof switches on sprinkler valves in mechanical room. McCarter Electric Change Order 5 in the amount of $3,163.70 - This change order covers additional cable in various sections of the building concerning the duct work, etc. MUSEUM Clancy and Theys - Change Order G-7 - in the amount of $ (10,886.00) This change orders covers installation of interceptor manhole in storm drain, paint stairs, installation of dryw~ll partition and deleting parking lot. . Barnes, Inc. - Change Order HVAC-6 - in the amount of $8,015.00 - This change order covers revisions to ductwork at various locations, relocating thermostats at various locations and adding diffuser and sidewall grille, including wall penetration. SENIOR CENTER PROJECT . David R. Polston - Change Order No. 1 - in the amount of $400.00 - This change order covers additional work for soil testing services at site work. SEWER Skipper's Well Drilling - Change Order No. 1 - in the amount of $300.00 - This change order covers the installation of chemtech feeder at 5326 Wrightsville Avenue. True Line Company - Change Order No. 1 - in the amount of $4,000.00 - This change order covers boring sewer services as needed. Rabon and Son's - Change Order No. 3 - in the amount of $3,000.00 - This change order covers asphalt repairs in the sewer services. Espey Huston and Associates - Change Order No. 1 - in the amount of $2,398.00 - This change order covers survey and design for a sewer line extension on Masonboro Sound Road near Grainger Road. This change order is a part of the mobile home parks contract (Contract 90-0134). . Espey Huston and Associates - Change Order No. 1 - in the amount of $3,947.00 This change order covers the horizontal surveying for field staking of sewer lines. This would make easy replacement of disturbed survey points. Espey Huston and Associates survey crews will keep field . The Contractor, Jacob Builders, submitted an estimated cost of $26,000 for this work. This would have been included in the Contractor's bid. This contract covers the collection system (90-0228). Espey Huston and Associates - Change Order No. 2 - in the amount of $25,675.00 - This change order covers additional surveying and redesigning of the sewer collection system. This is due to relocation of sewer line as required to work wi th owners in obtaining easements, and also working with. landowners to prevent condemnation. This contract covers the collection system (90-0228). Mark Johnson, Inc - Change Order C-4 - in the amount of $590.00 - This change order covers the additional cost of $590.00 for the 16" transition connection that was necessary to connect the 16" poly pipe to the existing 16" PVC force main at Bradley Creek. . Mark Johnson, Inc. - Change Order C-6 - in the amount of $3,391.60 - This change order covers the difference in cost between DIP and PVC carrier pipe which is $13.90 per foot. Mark Johnson, Inc. - Change Order C-7 - in the amount of $12,650.00 - This change order was necessary because CP&L buil t an access road into their transmission relay site after the project's contract had been let. This situation could not have been anticipated by the Contractor and is therefore not considered part of his original scope of work. Cost associated with this change is full and total compensation for removal and restoration of the roadway to it's previous condition. Sneeden, Inc. Change Order E-4 in the amount of $9,457.90 This change order is required due to unanticipated rock excavation at Bayshore Pump Station #2. The cost associated with this change arise from additional equipment use, special dewatering operations and increased safety measures incidental to rock excavation. . . County Commissioners Page Number Four January 4, 1991 STEAM PLANT Clancy and Theys - Change Order No. 1 - in the amount of $41,114.00 - This change order covers installation of 18" sleeve in turbine foundation wall for condensate piping, installation drain with piping in turbine foundation slab, change pavement in tipping floor area from asphalt to concrete, provide concrete equipment pads for pumps and other mechanical and electrical equipment, remove old stack to make room for new construction and installation metal double door on water building. Volund USA Change Order No. 2 in the amount of $13,230.00 This change order covers providing gallery platforms and ladders for access to the Nox system nozzles. Mallette, Director~ Department of Human Resource ~~~ Personnel Policy Amendment TO: RE: I am requesting that the attached amendment to the indicated section of the County's Personnel Policy be presented to the County Commissioners for their review and subsequent approval. A brief explanation of the effects of the change is discussed below: . Article VI Sec. 10 - Purchase of Surplus Property Restricted The amendment to this section will clarify the broader intent of the policy by defining more specifical~y, "county property"; encompassing the various means of disposing of such property; and, providing leniency in omitting, "members of their immediate family". Article VI Sec. 10 - Purchase of Surplus Property Restricted CURRENT POLICY County employees, public officials, and members of their immediate family are prohibited from placing bids on any personal property sold at a County auction. REVISION . County employees, are prohibited from purchasing any surplus county personal property of monetary value, whether purchased by the County or, donated to any representative or department of the County. This restrictions applies to all auctions and other sales events. ARM. DOC f' NORTH CAROLINA STATE DEPARTMENT OF TRANSPORTATION REQUEST FOR ADDITION TO STATE MAINTAINED SECONDARY ROAD SYSTEM North Carolina County of New Hanover Description: Springtime Road and Ripwood in Springtime Subdivision (Division File No. 663-N) WHEREAS, the attached petition has been filed with the Board of County Commissioners of the County of New Hanover ~equesting that the above described roads, the location of which has been indicated in red on the attached map, be added to the Secondary Road System; and WHEREAS, the Board of County Commissioners is of the opinion that the above described roads should be added to the Secondary Road System, if the roads meet minimum standards and criteria es.tablished by the Division of Highways of the Department of Transportation for the addition of roads to the System. NOW, THEREFORE, be it resolved by the Board of Commissioners of. the County of New Hanover that the Division of Highways is hereby requested to review the above described roads, and to take over the road for maintenance if they meet established standards and criteria.
