The state funds for the Wise Woman Program (12 hours per week) and the grant funds for the Good Shepherd Clinic (28 hours per week) together will support a regular full time PHN position with benefits. No additional office space is required, as this position works primarily in the off-site Good Shepherd Clinic and in existing space in the Health Dept. assigned to the Wise Woman Program. We request that the two vacant PE positions be combined into one (1) position, reclassified as a Physician, combining the existing salaries currently included in the adopted budget to fund the higher level classification for the Physician position. Human Resources did a salary analysis in the Spring 2007, resulting in an increase in the starting salary of the PE positions; however, that has still not resulted in viable candidates after multiple recruitment efforts. We do not have an adequate number of providers in our clinic at the present, and this is limiting our ability to provide clinic services. There are additional coverage needs beyond our core clinical services (child health, women's health, and sexually transmitted disease f services). Our physicians who have staffed our Neurology Clinic for many years have resigned from the clinic and we need backup for our Colposcopy Clinic and Behavioral Health Services. We would like to use the salary funds from both vacant positions to recruit a physician who can meet all our service needs. No additional county funds are needed beyond those currently budgeted for the two existing positions, which, when combined, will sufficiently cover the cost of the one Physician position. Recommended Motion and Requested Actions: To accept and approve reclassification of a Physician Extender position to a Physician position, delete one vacant Physician Extender position, and use salary already included in the adopted budget for the two (2) vacant PE positions to fund the reclassified position and submit to the New Hanover County Commissioners for their consideration. Recommendations Dave, At our meeting on 8/8/07, the F.A.C.T. members agreed to submit the following recommendations to you for your consideration: • International Travel Clinic medication: recommend increasing the cost of Oral Typhoid (bottle of 4) from $40.00 to $50.00, based on increase in product cost. • Hepatitis A vaccine: recommend expanding the categories of clients to be offered the free state-supplied vaccine at no cost to anyone interested, including international travel clinic patients, until the remaining supply of 300 doses is exhausted. This vaccine will need to be used before it expires. • Based on newly released Medicaid reimbursement rates, recommend increasing the fee for Newborn EPSDT Screen/HV (99502EP) from $65 to $90. We have not provided this service to date, but the cost of providing the service would require the increased fee, and Medicaid's increase in the reimbursement rate would allow the higher amount. • Establishing a fee of $15.00 for SED Rates (85651), covering the cost of this service and pricing it consistently with other similar laboratory services. • Changes to the NHCHD Fee Policy as recommended by the State Administrative Consultant during the recent monitoring visit (see attached documents). • One additional change to NHCHD Fee Policy, required as described in memo from Joe Holliday, Women's Health Branch Head, for compliance with federal Title X income guidelines. This change will exclude all family planning accounts from being sent to the NC Debt Set-Off program, as it is seen as a barrier to service. This change has been incorporated into item "K" in the fee policy attached to this document. Each of the requested changes are expected to affect revenues at an amount less than $5,000 per year, permitting Health Director approval.
