C'R'E'ECJ--{, 'R:N" 'DecemfJer 2003 WHEREAS, Betty Creech retired from the New Hanover County Health Department after more than 29 years of dedicated service as a Public Health Nurse, Public Health Nursing Supervisor and Public Health Nursing Director; and WHEREAS, she began her public health career as a Public Health Nurse Trainee in 1973 and rose through the ranks to a Public Health Nursing Director; and WHEREAS, she exhibited unwavering loyalty to public health and resolved to ensure the smooth operation of the Community Health Programs; and WHEREAS, she has been a mentor to many nurses with her calm demeanor and a stabilizing strength during troubled times of natural disasters, staff shortages and disease outbreaks in the community; and WHEREAS, she implemented and planned numerous programs during her tenure such as Orthopedic, Neurology, Kindergarten Health, Well Baby, Nursing Clinics, Disaster Health, School Health and the Jail Health programs; and WHEREAS, she was recognized as an expert in disasters, serving on committees with the Cape Fear Chapter of the American Red Cross representing New Hanover County Health Department, and developed the first Emergency Policy and Procedure Manual for the Health Department, and WHEREAS, she was a recipient of the Margaret B. Dolan award from the nursing section of the North Carolina Public Health Association; and WHEREAS, she served as President in 1999 and on the Governing Board of the North Carolina Association of Public Health Nurse Administrators, and on the North Carolina Board of Nursing Continued Competency Committee; and WHEREAS, she demonstrated commitment to public health and an outstanding work ethic by coming to work early, staying late, and canceling vacation leave to unselfishly serve the citizens of New Hanover County; and WHEREAS, she inspired her staff, co-workers and residents of New Hanover County with her leadership, her love of God and family, and her tireless efforts to promote public health for all; be it therefore RESOLVED, that the North Carolina Association of Public Health Nurse Administrators go on record as honoring Betty B. Creech, and that a copy of this resolution be presented to her; and be it further RESOLVED, that a copy of this resolution be sent to the North Carolina Association of Public Health Nurse Administrators; Dr. Joy Reed, Director of Public Health Nursing and Professional Development; W. Article I, Section 5-3(7) is hereby amended by deleting subsection (7) and substituting the fol1owing: (7) The director and all animal control officers are empowered to issue citations to any person if there is probable cause to believe that such person has violated any of the' provisions of this chapter. Citations so issued may be served in person upon the violator by the director or any animal control officer or they may be mailed by certified mail. Any citation so served or mailed sha1l direct the alleged violator to make payment of the fine provided therein to animal control on or before a specific day and hour stated on the citation and the period so specified shall not be less than 72 hours after service or delivery to the vioJator. If the violator is served by mail, thtl vioJator shall have 20 days from the date of the citation to pay said citatiOD. The citation fines may be recovered by thtl county in a civil action in the nature of a debt if the offender does not pay the pena1ty within 20 days after being cited. We have been able to increase the cost per participant to $11.50 per month. A caseload increase bas also been issued to counties that have an average caselolld based on the first four months of the SFY that exceeds their 2003 base. In addition. funds are being added to your budgets for new or ,,"hanced breastfeeding support and/or promotion activities. Attached please find 2 original agreement addenda, a budgetary estintate, and a budget revision form. The per participant and caseload increases have been allocated under activity #5403. Local agencies may distribute this funding according to their needs across the four activities. However, the breastfeeding funds have been budgeted in activity #5409 and serve as the minimum amount that can be budgeted in this activity. . We are requesting approval to delete all references to the JailMedical Health Program. 2. Additionally, we are requesting approval for the following changes as they relate to services provided within our Personal Health Services Division. Note: Changes I additions are shown in bold priilt: New Hanover County Health Department Fee Policy , I. General Guidelines F. The :New Hanover County Health Department (NHCHD) will require ''proof of income" to reduce charges when applying the sliding fee scale. If a patient is unable to produce this required information, they will be placed on a 100% sliding fee scale status~ror a period of ten (10) days. If proof of income is provided within the ten day period, the patient will be billed accordingly. If proof of income is not provided within the established timeframe, the patiellt will be billed at 100%. The NHCHD representative has the right to verify income information in all cases, however the patient must read, understand, and sign the income statement (page 30) in omerfar their income to be checked. The sliding fee scale does not apply to all services. Services with flat ~te fees do not require proof of income. In extreme or unusual circumstances, the Health Director or designee may make exceptions.. H. Payment, or co-pay for third party billing, is expected at the time of service for all chargeable services. Partial payment is accepted for all chargeable services, with the exception of vaccines not provided by the state. Co-pays As of 01118104 23 e are not subject to sliding fee scale. Medicaid patients are exempt from co-pays. , , I. If a patient has a remaining balance on their account, a payment agreement and schedule will,be established and signed by the patient (page 31). Patients who have demonstrated no "good faith" effort.to pay may be subject to service restrictions as allowed by law. Maternity Care Coordination, Child Sen1ce Coordination, Maternal Outreach, Intensive Home Visiting and Parenting Classes will he billed to Medicaid. For Non-Medicaid patients, these services will be billed to state grants. Orthopedic and Neurology clinic services are available only to Medicaid eligible and indigent patients. All Non-Medicaid visits will be billed to state grants. O. Reimbursable visits, for patients with insurance coverage, will be billed to the _ insurance company. If there is a balance remaining after the insurance ,., payment is,received by NHCHD, the balance will be billed to the patient based on thelr established slidiJ1g fee scale rates. S. New Hanover County employees may purchase certain in-stock medications, at a reduced priee, for themselves, their spouses, and their dependent children. T. All clinic and laboratory fees will be collected 88 part of the check-oat process by the New Hanover County Health Department Billing Clerk. II. Program Specific Information A. Environinental Charges- Water Bacteriology 2. On an as needed basis. the Environmental Health clerk will submit a report to the Laboratory Director listing total water sample revenues for the designated time period. 3. Charges may be made for supplies not required by the plan of contraceptive care based on cost of supplies. Charges for extra cycles of pills may also be 'made. Charges for family planning services, to include supplies, will be billed based on sliding fee scale. However, non-family planning services will be charged according to locally established fee schedule and will apply to all patients (Pages 24-28). ' C. Laboratory 2. The patient will be directed to the billing cierk. D. ChDd Health Servi~ 1. Children seen for Child Ilealth Services will be charged in accordance with the NHCHO Sliding Fee Scale (Page 25). e m. Accounts Receivable The Accounts Receivable Bookkeeping System includes: 1. The fee policy will be explained to each patient with explanations of purpose and details of procedure when the patient presents for services. Each patient is given an opportunity to pay and every effort will be made by the staff to collect total or partial payment on the day of the visits. Payment in fuR is required for flat fee services. 2. ProvIded that patient confidentiality is not jeopardized, bills showing total charges Qess siiding scale discount) will be mailed to patients 30 days after their visit Two additional statements with balance owed will be mailed if no payment or subsequent visit is made. 3. There has been no intervening charge visit within 13 months and the patient sti1l wishes to remain an active patient Future services may be denied if effort for payment is not made. 2. Small amounts are being paid toward the bill. An itemized list of uncollectahle outstanding patient balances will be prepared at the end of the fiscal year fur the Health Director's review. Those approved by the Health Director and the Board of HeaIIh Win be writlm oft: The Accounts Receivable system shall indicate the recording of the bill as uncollectah1e by adjusting die patient balance to zero. Evidence shall be on file to document required billings. A self-pay patient (categorized as a 60"10 or greater on the sliding fee scale) with a previous bad debt write off. will not be alloWed tocbarge services unless aJ>aymeol is made toward their previous baIanc:lI and odIIr payment arrangements are agreed upon. A self-pay patient, categorized as 40% or beloW on die sliding file scale, will not be denied services but their account will re reactivated. . If a patient retwns to the health department after a bad debt has been determined uncollectahle their bad debt write off shall be reactivated and the billing process resumes. The entire workbook will be available for your review at the New Hanover County Board of Health Executive Committee on January 27,2004. e Our total Budget Request FY 2004-2005 is $11.728.929. This is a projected increase of $ 290,789 (2.54%) over the Adopted Budget FY 04 amount of $11,438, 14Q. Under the direction of the County Manager, no new position,s are being requested for FY 04-05. Revenues through federal and state grants, fees, Medicaid, and other sources show an increase of $ 284,881 (4.79%). The net increased cost to the county is $ 5,908 (.11%). This budget request does not include projections for possible cost increases in fringe benefits (insurance), salary market adjustments or merit. Our Budget Request for FY 2005 is due to the County Budget Office on February 13, 2004; for review by the County Manager, Allen O'Neal, and his Budget Committee. In May we will receive the County Manager's Recommended Budget for FY 2005 for the New Hanover County Health Department. General consensus was to keep these animals from being allowed to be housed in a residential location in NHC. We have concerns as a staff about the addition of a potential rabies vector, and who would police after the animals if they were to get loose. The couple, Monica and Howard Loughlin, though appearing very responsible, cannot guarantee that their pets will not present a danger to the county's citizens. Attached Is a copy of the minutes from the ACS Advisory Committee held on January 21, 2004, where the Loughlins pleaded their case. The Advisory Committee had the same concerns as ACS staff, specifically the threat of adding another rabies vector to the animal roster in NHC. They questioned the couple's ability to have insurance coverage in case of damage by the bears. They had no immediate answer The Committee also was concemed about county liability, if we are the overseeing group granting permission for the bears to reside here. They are seeking the right from the Board of Health to house their pets in NHC. Also attached is the BOH Wildlife Regulation, in particular, Section Two regarding maintenance of wild or exotic animals. There are a number of lettars and signed petitions from neighbors to support their right to house the bears here. Zoning, the USDA, and the Wildlife Commission have no restrictions for them to keep them here (see letters). Their veterinarian Is Dr. Bill Rohn, from Port City Animal Hospital. The bears are twins, bom at Totem-in-Zoo. They have been neutered and front paw declawed. The species Is not indigenous to North America. The Loughlins would like to attend the February BOH meeting to discuss this with the Board. The additional documents will be available at the Health Department I have left a voice-mail message with county attomey, Holt Moore, requesting his attendance at the Executive BOH meeting Tuesday evening. Thanks to Gretchen, John, and other volunteers to make it happen. It was successful enough to consider it a continued effort. Approximately 45 animals were vaccinated. 6. Seized animals (this year's!) We went to court on January 12,2004 (all day!). We go again on February 3, 2004, since the judge was not satisfied with the plea agreement. The animals will probably go to various rescue groups, if they become our property. 44 e . . 7. SIN reimbursement fund from state. I have joined a group called Citywide Neighborhood Network (CNN). DSS personnel are also members, and I have talked with them about implementing this program. Volunteers welcome to assist! New Business 1. Title change. I am now Animal Control Manager, as opposed to director or public health division manager. Another portion of the organization changes in the health department. 2. Wildlife as pets in NHC. Discussion on whether or not these can be approved to be kept at a residential household in NHC by permitted approval. Reference wildlife regulation per Board of Health. The group first felt the bears were outside our jurisdiction, because they are non-domestic animals. However, the BOH regulation stipulates about the maintenance of wild or exotic pets in NHC. Conclusions: Concerned that we would add another rabies vector to the ones already in NHC. Also, liability responsibility to NHC, if the bears escape. The prevalence of rabies virus in our community was a serious concern. 3. Hearings appeals process. Discussion on procedural coverage and related issues. Group is pleased with our proceedings. Suggested to Ms. Goldston to continue calls to ACS, if the dogs are viewed running loose again. 4. Ordinance change. Must have approval from Advisory Committee to present to BOH. Deals with computer-generated citations. Must be mailed certified. Group gave unanimous approval to change. 5. HD accreditation process. Minimal provision for animal control agencies. Good idea to send info to group to add more ac-related criteria to document. Please include mandatory spay/neuter of all adopted pets as a criteria. Also, some means of education and a euthanasia protocol. 6. Adoption outreach program. Consider implementation after SIN facility complete and operational. Pets would be taken off site and adopted there. Some possible locations are PetsMart and Pet Supermarket. Animals must be neutered prior to starting this practice. 45 e - - . . . 7. I have been involved, on a small scale, with the care of the Himalayan Black Bears under the care of the Loughlins. The bears have come into our hospital several times to assess their body weight, and two or three times for veterinary care. The bears have not shown any tendency toward aggressive behavior and were relatively easy to examine and treat. They have interacted with numerous people on these occasions, most of whom the bears had never encountered previously, and have reacted to the attention of those people in a very positive manner As you know, bears are omnivorous and prefer to eat plantslfruitslvegetables. The recent incident of a lion (?) mauling and killing a child, though unfortunate, is an entirely different situation. That animal's instinct was to hunt and kill another animal. I know the Loughlins have taken several measures to ensure the safety of the general public as well as the safety of the bears. The Mure educational opportunity the Loughlins are offering our community should be looked upon in a favorable manner These animals are nearing endangered status; Howard and Monica are providing care and a home to a few species in need. We leamed that Howard and Monica visited the adult Himalayan black bears at Tote-Em-In Zoo every Sunday to bring them pies, fruit and rawhides as a treat The Loughllns were quite fond of the offspring as weU. So fond that they purchased the twin cubs that were bom last January and donated them to us at Little Man's Zoo. They wanted to keep the family of bears close to home. From the end of March until the first of November, the Loughlins would travel to Chadboum every Friday to pick the twin cubs up and take them to their house for the weekend. As the cubs grew, the Loughlins wanted to have a bigger, more secure facility for the bear cubs to reside when the cubs visited on the weekends, so the Loughlins built a zoo-type facility. When we saw the facility that the Loughlins had built, we were amazed at the construction. The enclosure is one of the best enclosures that we have seen in the zoo business. We were so amazed that we donated the bear cubs to the Loughlins. Teddy and Baloo (bear cubs) are like the Loughlins children. The time the Loughlins have spent with Teddy and Baloo is astonishing. The Loughlins have provided a safe and healthy environment for Teddy and Baloo as well as for the public. Small populations live in Taiwan and the Japanese islands of Honshu and Shikoku. Physical Appearance The Asiatic black bear is a medium sized bear. The males are about twice the size of the females, with the males weighing 220 - 440 Ib (100 - 200 kg), and the females weighing from 110 - 275 Ib (50 - 125 kg). Their fur is uniformly black, with a lighter brown muzzle, and a distinct white chevron (V-shape) across its chest. They have large rourided ears that set low on its head. They have relatively short hair all over their body except for a large mane of hair that sticks out of the sides of its head. Like all bears, the black bear is plantigrade, which means that it walks with its entire foot like a human, rather than on its toes like dogs and cats, which are digitigrade. They can also stand up in their hind legs for extended periods of time, which makes them look rather human. Bears also tend to sit down on their rear with their upper body off the ground, like a person, too. They have a very short, stubby tail, just like all bears. Diet The Asiatic black bear is primarily herbivorous. They feed on different parts of plants at different times of the year to extract the most nutrients from those parts. They feed on various nuts, raspberry canes, cow parsnip, butter bur and hydrangea in the springtime, fruits, berries, and bamboo shoots in the summer, and acoms, hazelnuts, chestnuts, pine nuts, beech nuts, and walnuts in the autumn. Reprodnction and Social Behavior Mating season is from June to July. but in the southem areas of their range mating can be as late as August. The pair stays together for only a few days during the breeding season, and during this time they copulate many times. Female bears are induced ovulators, meaning they have to be stimulated several times in order to ovulate, so this is why they must mate so often. Also, like other mammals in the order camivora, the male bears have a special bony structure in their penis called a "baculum" This structure serves to stimulate the female into ovulating, as well as prolonging copulation by widening to lock the two mating bears in a copulatory "tie" This tie may last for 10 - 30 minutes. The cubs are bom in a den during the winter denning months, usually in January Two or three cubs usually comprise the litter, and they weigh 10 oz (300 g) when they are bom. Bears, unlike any other mammal, are born very small, much smaller than mammals of a similar size. In fact, when they are born, their weight is 10% of that of a mammal of similar size. The reason bears are born so small is because their mother fasts (does not eat) during the gestation period (pregnancy) with her young, unlike any other mammal which continues to eat while pregnant with her young. The mother bear's body proteins actually break down to provide glucose in order to nourish the young while they are still inside her And since the baby bears obtain very little nutrients while still inside the mother, they are bom premature. To make up for the lack of nutrients during gestation, the milk of bears is more rich in nutrients than other camivores. Because the milk is so rich in fats and nutrients, young bear cubs do not need to drink as much milk as other carnivore cubs. Like most bear species, the Asiatic black bear dens in wintertime, since food is relatively scarce. Besides this fact, very little is known about Asiatic black bear behavior It is assumed that it is similar to the American black bear, being solitary and territorial, but until more information is gathered about this bear, little will remain known about its social habits. Threats The Asiatic black bear is suffering mainly from human encroachment on their territory, as well as being hunted for their body parts. Subspecies Initially there was believed to be four subspecies of Asian black bear: U.t. japonicus (Japan), U.t. fonnosanus (TaiWan), U.t. ussuricus (Mongolia) and u.t. thibetanus for all other regions. Now, all these subspecies are considered to be just racial diversities, and not separate subspecies. Now, there is believed to be only one true subspecies: u.t. gedrosianus (southem Pakistan).
